Create a Fillable PDF Form

Make an existing PDF fillable. Draw text boxes, checkboxes and dropdowns on the page.

Drop your file here

Turn a regular PDF into a fillable form. Draw fields where people should type or tick, give them names, and download the result.

How to use Create PDF Forms

  1. Choose or drop the PDF you want to make fillable.
  2. Pick a field type above the page: Text box, Paragraph, Checkbox or Dropdown.
  3. Drag on the page to draw each field, and click a field to rename it, edit dropdown choices or delete it.
  4. Click Create fillable PDF and download the file.

What it does

  • Four field types: single-line text box, multi-line paragraph, checkbox and dropdown.
  • Set dropdown choices as a comma-separated list.
  • Move fields by dragging them, and see all fields in a list with their page numbers.
  • Add fields on any page, using the previous and next page buttons.
  • Works on PDFs that already have a form: new fields get unique names so existing fields are kept.

Good to know

  • Radio buttons and signature fields are not available as field types.
  • A field's size is set when you draw it; to change it, delete the field and draw it again.

Your files and privacy

The PDF is opened and the fields are added in your browser; the file is not uploaded to Docshar's server. If you are signed in, Docshar counts your operations (tool, number of files, sizes and page counts), never the file contents. See the Privacy Policy.

Questions

Can the form be filled in other PDF readers?

The fields are standard PDF form fields, so PDF readers that support forms can fill them in.

Can I test the form here?

Yes. Open the downloaded file in the Fill and Sign tool to see and fill its fields.
